Who we are
System Change Partners is helping communities use data to drive better system outcomes. We work with community champions to design tools that bring real-time insight into what’s working, what isn’t and how to create actionable steps for change. We coach communities to build data systems that work for them.
With over a decade's experience in homelessness data systems, data science, and evaluation, Liz and Vishesh created SC Partners to fill a glaring gap in our field. Communities are passionate about systems change but too often don't have the data infrastructure available to help them make sound real-time decisions.
How can you get to your destination if you don't know where you're going?
SC Partners helps communities create measurable goals so that they know where they're going and supports them to build the road map (and infrastructure) necessary to get there, while tracking their progress along the way.
Our Approach
Our approach has 3 simple steps:
Bring fragmented data together to create a real-time picture of the system at an individual and birds-eye level
Create simple tools and infrastructure that gets data to the right people at the right time, in order to accelerate positive outcomes and reduce disparities
Build community capacity to gather and respond to the data themselves

Liz Harding Chao (M.P.A.)
Co-Founder
Liz (she/they) believes that the answers to society's most complex problems lie within communities and impacted people themselves. Liz is an expert in working with stakeholders to conduct evaluations and research that center racial and LGBTQIA2S+ equity and the voices of people with lived experience. She has authored multiple key reports that significantly advance the knowledge base in the field of youth and young adult homelessness and is frequently invited to speak at conferences and on panels about their research. Liz holds a Master of Public Administration from the University of Washington and is currently a 2L at the Georgia State University College of Law.
Vishesh Jain (m.S.)
Co-Founder
Vishesh (he/him) is a seasoned professional with over 7 years of dedicated experience in the United States, working closely with public sector organizations. His passion lies in the intersection of technology, data, and social impact, where he has consistently demonstrated a commitment to build actionable data infrastructure and foster strategic partner relationships. Vishesh specializes in crafting and implementing data-driven solutions that optimize system processes. He believes in harnessing the potential of machine learning algorithms and data visualization practices to create solutions that make a difference in the world
